My SST Portal
My SST Portal ouvre une nouvelle fenêtre is our secure platform that you can use to fill out and submit your appeal forms online. You can also use it to manage documents throughout your appeal process.
You can use the portal to:
- Fill out and submit your appeal forms
- Submit supporting documents
- Receive documents from us
- Keep all your appeal documents organized
- Communicate with us
You can complete appeal forms on the portal as a guest. For everything else, you must create an account.
Choose the form you need from the options below.
Forms for level 2 Employment Insurance (EI) appeals
Level 1 - If you want to appeal a reconsideration decision from Service Canada, go to the EI Board of Appeal opens a new window.
Level 2 – Appeal Division
You can only use this form to appeal decisions from:
- the Employment Insurance (EI) Board of Appeal, or
- the SST’s General Division.

Forms for Canada Pension Plan (CPP), Old Age Security (OAS), Guaranteed Income Supplement (GIS) and Canada Disability Benefit (CDB) appeals
Level 1 – General Division
Fill out this form to appeal a CPP disability, other CPP, OAS/GIS or CDB reconsideration decision from Service Canada. The button below will take you to our secure My SST Portal platform where you can fill out the form as a guest or create an account.

Level 2 – Appeal Division
Fill out this form to appeal a CPP disability, other CPP, OAS/GIS or CDB decision from the SST’s General Division. The button below will take you to our secure My SST Portal platform where you can fill out the form as a guest or create an account.

Witness information form
If you’re bringing a witness to your CPP disability, other CPP, OAS/GIS or CDB hearing, fill out this form to tell us about them. You don’t have to fill out this form for EI appeals unless you’re bringing a witness to a hearing about a constitutional question, like a Charter challenge.
Only some kinds of appeals have witnesses. You don’t have to bring a witness to your hearing. Learn more about what witnesses do and the types of appeals that allow you to bring them.
This form isn’t currently available on My SST Portal. Instead, you can choose to fill out either the HTML or PDF version below. HTML is best if you use a screen reader. PDF is best if you want to print it and fill it out by hand. Learn how to fill out and send your form.
Other requests

For any other requests, you don’t need a form. You can reach us by email, mail, phone, fax, or TTY to:
- change, remove or add a representative
- send us more documents related to your appeal
- change your address or contact information
- ask for an interpreter
- ask for an accommodation if you don’t want to use the accommodation request form
- ask to be added as a party to an appeal
- ask questions about the appeal process
