How to complete and send us your PDF appeal form
Can’t open a form? Right-click on its link. Depending on your browser, click on either “Save target as…” or “Save link as…” to download to your computer. If you still have issues, contact us and we’ll help you get the form.
We recommend keeping these instructions open in a separate window so you can refer to them while you complete your form.

Completing a PDF form outside of the portal
If you prefer to use our PDF forms, you can choose to:
- print the blank form and complete it with a pen, or
- complete the form electronically
If you choose to print the blank form and complete it with a pen, make sure you keep a copy for your records. You can send it to us by email, mail or fax.
If you choose to complete your PDF form electronically, the information you enter won’t be saved online. You need to save the form to your computer or device, or the information you enter will be lost.
How to complete and save your PDF form electronically
These instructions may vary depending on your browser or your device. Contact us if you need help.
Step 1: Complete your PDF form
- Open the PDF form in your browser.
- Complete each section. Give all the information the form asks for.
- To sign your form, choose one of these methods:
- Type your name in the signature box
- Draw or insert your electronic signature (some PDF readers have a signature function that lets you draw or insert an electronic signature)
- Go to Step 2 and save the PDF to your device right away. If you don’t save it to your device, all the information you entered will be lost.
Step 2: Save the PDF form to your device
- Hold down the “Ctrl” (control) button and press the letter P to access the Print menu.
- In the Print pop-up window, find either the “Destination” or “Printer” dropdown menu, and select “Save as PDF.”
- Click the “Save” button at the bottom of the Print pop-up window.
- You’ll be prompted to choose a location where you want to save the form on your device. Choose a location and click “Save” again.
- You should now be able to find your form where you saved it.
Step 3: Send us your PDF form
Send us your form by email, mail or fax. Email is fastest.
Keep a copy of your form for your records.
Ways to send us your PDF form
If you completed your PDF Notice of Appeal or Application form electronically, attach your saved form to an email, along with any supporting documents, and send it to us.
If you printed and completed your form with a pen, you can use a scanner or a scanning app to scan your form so you can attach it to an email.
Make sure you send your documents in a format we can open.
